• Model Number (can be found on the test certificate or the rating plate at the rear of the unit)
• Serial Number (can be found on the test certificate or the rating plate at the rear of the unit)
• Date of Test (can be found on the test certificate or the rating plate at the rear of the unit)
• Date of Purchase
• Location of units (if different from your contact address)

Once we have your details on file we will contact you approximately 6 months from the date of test / purchase to arrange your FREE Service Health Check
